Fee Schedule
New Perceptions operates on a sliding fee scale; no one is refused services because of his/her inability to pay. For clients who are uninsured or underinsured, the sliding fee scale takes into account income, family size, other financial obligations, and number of sessions per week.
Clients are responsible for forwarding all completed insurance forms to the New Perceptions office before their third visit. If there is a secondary insurer we will require information on that plan as well. Authorization or referrals from HMO's may also be required.
Any change in a client's payment status during the course of treatment, such as a change in income or insurance, must be reported to our office as soon as possible.
