General Assistance Intensive Case Management
Willimantic Outreach

Case Management and referral services are provided by the ERSC, GAICM, & Willimantic Outreach. The purpose of these programs is to enhance client access to the Eastern region case management system and to provide services to high utilizers. Perception Programs employs 4 case managers to service these 3 programs. This case management system creates a more efficient client focused system that improves the quality of care and service provided. This includes transportation to: detox, residential treatment, court, medical appointments, Department of Labor and to recovery meetings. Primary services include outreach to clients and crisis intervention.

ERSC Porgram Update 2009

Case management services are provided through the ERSC, with the GAICM and Willimantic Outreach workers being specialty areas within the system.  These case managers enable clients to have a consistent support person as they navigate through different levels of treatment care and then transition into the wider community.  They also improve treatment retention and decrease emergency service needs.  During the last fiscal year, our four case managers provided 2,947 hours of service to 301 clients.
